Locating the Questionnaire link

To use the online questionnaire facility you will need to search for the case you wish to submit the questionnaire on.

When the case is retrieved the following screen will be displayed:

Click on the reference number link to access the Case Summary screen.

Once in the Case Summary screen, scroll to the bottom of the page to locate the documents section.

Click on the link to access the View Documents page.

Scroll to the bottom of this page to locate the link to submit a questionnaire:

When you have clicked on the link the following screen will be displayed:

Enter your LPA ID and Password in the fields provided and click on the Submit button.
