Continuing with a saved Questionnaire

To complete a previously saved questionnaire, locate the saved version of the form. 

This should be within the folder which was selected as part of the save process. Or you may have been sent it as an attachment to an e-mail.  Double click on the questionnaire to open.

The following screen will be displayed:

Click on the “resume appeal” button to continue adding information and/ or any attachments to the questionnaire.
