Submitting the Questionnaire

Before submitting the questionnaire, please ensure you read the information in the submit box. 

We recommend that you save the form before you submit it.  That way if anything does go wrong, you won't have to start again from scratch.

When you are sure that you have completed the form correctly, click on the submit button, the form will be checked by the system. 

A page will be displayed which outlines what will happen once the questionnaire has been submitted successfully.   This is your last opportunity to make any changes to the form.  If you want to change anything, click on cancel, otherwise, click on the OK button to complete the submission.
